A single person making less than $12,000 a year and a married couple earning less than $25,000 a year may be eligible for Medicare Part D. If an administrator for the Medicare program does not receive any additional information, they review cases on a yearly basis. If you report additional income, you may have to reapply if your income level changes to fall within the income guidelines for low-income Medicare coverage again.

If the decision is positive, a Part D recipient or recipients may never receive a notification that the extra help receives in paying for Medicare has been continued. The notifications will usually come in the mail at the same time each year. A recipient may never have to reapply for extra help to pay for his Medicare Coverage.
Circumstances Which You Must Reapply for Extra Medicare Help
If Medicare determines that you are no longer eligible and you disagree, you must reapply immediately. The review board that makes the administrative decision will require proof of your income, proof of your spouses income if you are married, and proof of the identity for the people on the application. This must happen shortly after you receive a notification of a review determined you are not eligible for extra help with your Medicare premiums.
Applying for Extra Help for Drug Costs
Extra help for drug costs must be considered on a case by case basis. You must apply for extra help with a drug any time you might need extra help paying for your prescriptions. The additional coverage ends when it can be determined that the prescription is no longer needed. If a person needs help with a different prescription, he must reapply to receive extra help from Medicare.
