How Do I Get A Medicaid Application?
To get a Medicaid application you will have to go to http://www.cms.hhs.gov/ or your states website and click the Medicaid link. Medicaid applications are different for every state, so make sure that you apply for the right kind to get the coverage that you want or need.

There are several different types of Medicaid. Some people believe that CHIP (Children health care) or Medicare are the same thing, but they’re not. You will need to research most of them to find out which one will best fit your needs. Some health care providers only accept certain types of Medicaid, so if you have found a provider that you like that accepts Medicaid be sure to call and ask what type they accept for payment.
Applying for Medicaid:
After you have carefully reviewed all of the Medicaid options you will need to apply. There are several ways to do this including online or by mail. They will need proof of everyone in your houses income along with your social security number and other various forms of identification.
Waiting:
The Medicaid application typically takes 30 days to be processed. However it may be longer or shorter, once again applying varies by state.

If you had already checked with your health care provider to make sure they accept Medicaid, bring your Medicaid card to your next visit, or your child if that’s who you were applying for. If you want to find a new provider; call a number that will be on your form to find a health care provider near you.
